Required paperwork:
- Player Registration Application – completely filled out, signed and dated.
- Player’s Birth Certificate – original for verification plus a copy to turn in.
- Current player’s picture – does not need to be taken by a professional.
- Proof of residency – such as a utility bill.
- Registration Fee – cash, money order or check (addressed to EVYSL).

Additional Fees:
- A $10.00 non-resident fee will be assessed if player does not reside in Lake Elsinore.
- A $25.00 fee will be charged for returned checks.
Additional Information:
- Each player will receive one free LA Galaxy game ticket.
- We provide full uniform, LIGHTED FIELDS, pictures & a trophy for each player.
- Price also covers registration, insurance, referees and field maintenance.
- Practices will begin in August to prepare all teams for the first Fall Season game on September 8th, 2012.
- Only complete registration packages will be accepted, no exceptions. Forms can be obtained and turned in at Alberhill’s Snack Bar, or you can download the form.
